Our Primary Care Practitioners provide high quality, comprehensive medical care and treatment to a diverse safety-net patient population in a patient-centered, team-based model with an emphasis on the prevention of disease, promotion of health, and management of chronic disease. The clinical team includes certified medical assistants, RN care coordinators, clinical pharmacists, health navigators, and behavioral health consultants.

To allow the time to deliver high quality care to patients with complex needs, our practitioners manage panels between 700 and 1,200 patients and see about 14 patients per day. After-hours call is shared among the full primary care group and supported by a nurse triage line that manages the vast majority of after-hours requests. Our providers have the option of teaching medical residents from several organizations as well as medical students from the local medical school.

Community Health Centers of Benton and Linn Counties (CHC)
The CHC opened as a Federally Qualified Health Center (FQHC) in 2004. It is the medical home for over 9,000 patients across five clinics and two counties.

The CHC provides physical, behavioral, dental care, and medical treatment for substance use disorders. The CHC works closely with Benton County Public Health, County Mental Health programs, and school districts at our two school-based sites.

The CHC utilizes the clinical team to support patients in accessing health. We are committed to improving health outcomes through a medical home model with a strong focus on prevention and disease management. The CHC sites are all Patient Center Primary Care Home (PCPCH) certified at a high level. Our organization serves communities with Health Professional Shortage Area (HPSA) ratings between 10 and 13 (https://data.hrsa.gov/tools/shortage-area/hpsa-find) and qualifies for National Health Scholars, National Health Service Corps and other Loan Repayment Programs.

    • $98,378 - $133,779
    • This position is eligible for up to two salary raises within your first year. An Increase may be given at the end of the 6-month probationary period, depending on starting salary, and at the 1-year anniversary, as long as your performance is in good standing! Annual performance-based increases are given until the maximum of the salary range is reached.

Minimum Qualifications for Physician Assistant (HCP5)
  The following minimum qualifications are required for this position:

  • Physician Assistant Unrestricted Oregon Medical License (license to practice in the State of Oregon or licensed in a different state with the ability to obtain an Oregon license).
Applications will be accepted without current Physician Assistant Unrestricted Oregon Medical License but offer and start date will be contingent upon obtaining Medical License. Must be able to obtain Physician Assistant Unrestricted Oregon Medical License within 3 months of job offer.
