Location: Total Health Community Clinic - Medical | 180 Atwater St N, Monmouth, OR 97361

Clinic Type: Outpatient Community Health Center / Federally Qualified Health Center (FQHC)

 

Position Status: Flexible Part-Time or Full-Time (4-10's or 5-8's)

Office Hours: Monday – Friday | 7:00 a.m. – 6:00 p.m.

Sign-on Bonus: $21,000 (Internal candidates not eligible)

Compensation:
Provider compensation is salary-based. The base salary for a Physician (MD/DO) ranges from $188,656 to $235,872, with potential to earn up to $283,088 based on experience.

YOUR ROLE:

Are you passionate about increasing access to high-quality healthcare for underserved populations? If so, this is your opportunity to join our primary care team at Total Health Community Clinic.

As a Physician/Primary Care Provider, you will work with an integrated team of medical, dental, mental health, and community health professionals to provide holistic care to diverse patient populations, including the homeless, LGBTQIA+, refugee communities, and more. You will be responsible for providing direct patient care, coordinating care plans, and ensuring a smooth continuum of care for patients with complex needs.


YOUR TEAM:

SPECIFIC DUTIES:

  • Oversee patient care coordination, including specialty referrals, case management, and enabling services.
  • Perform professional medical services, including examinations, diagnosis, and treatment planning.
  • Collaborate with nurse practitioners and physician assistants and serve as a supervising physician for up to two mid-level providers.
  • Provide ongoing patient care, consistently meeting productivity and documentation standards.
  • Actively engage in organizational initiatives such as 340B, Patient Centered Medical Home Certification, and Meaningful Use requirements.
  • Work collaboratively with dental and behavioral health teams to provide integrated care.

QUALIFICATIONS:

  • MD/DO with board certification in Primary Care (Family Medicine, Internal Medicine, or Pediatrics).
  • Licensed to practice in Oregon with prescriptive authority and DEA.
  • Ability to be professionally bondable and insurable.
  • Preferred: Experience with obstetrical care and bilingual skills (English/Spanish a plus).
  • Valid Oregon Driver's License and CPR certification required.
  • Experience in community health is a plus.
  • Team-oriented with excellent communication skills and the ability to work independently.
