Family Nurse Practitioner – Community-Focused Primary Care

We’re seeking a dedicated Family Nurse Practitioner (FNP) to provide primary care and health promotion services in a collaborative, patient-centered environment. What You’ll Do
Provide primary care services including well-child visits, school physicals, immunizations, and chronic disease management
Conduct health education and promotion for individuals and groups
Administer or prescribe treatments and medications as appropriate
Order, perform, and interpret diagnostic tests and lab work
Evaluate and treat minor illnesses and injuries
Maintain accurate and timely documentation in medical records
Refer patients for specialty services when needed
Perform procedures within scope of competency
Uphold strict patient confidentiality and clinic protocols
Collaborate with medical leadership on additional services as needed
Serve as a key Family Nurse Practitioner (FNP) resource for patients navigating chronic conditions, preventive care, and community health services.
What You Bring
Completion of MSN and accredited Family Nurse Practitioner program
Current Oregon license to practice medicine, DEA license, and CPR certification
Current BLS certificate
Strong communication and teamwork skills
Ability to work independently and collaboratively in a fast-paced setting
Commitment to confidentiality and culturally responsive care
Willingness to work occasional evenings and weekends
Experience in FQHC or underserved settings preferred
